I have been on 40mg of fluoxetine a day for the past 2 years. I have experienced a severe episode of anxiety/depression recently and my psychiatrist thinks that the medication may have "bottomed out". Is that possible with fluoxetine and is duloxetine a safe or viable alternative?

My psychatrist recommended that I try cymbalta, but I wanted some other opinions first. What are the long-term and most serious side-effects of cymbalta?

You should research the effects of duloxetine, which can be found online. Some people do experience a loss of effect from an antidepressant, which may indicate the need for a more careful diagnostic evaluation. No one medication is any better than any other medication. All medications work for some people; no medication works for all people.
