I know I have ADHD, but everyone has issues with my dosage.
Isn't every person's metabolism different? And is there a way to prove I need a higher dosage than I am being prescribed, since at the dose I am on, I can't remember how why I left the kitchen to go to the bedroom? |

Is it possible to suffer from ADHD and substance abuse? What treatment is available for dual diagnosis? |

Anorexia |

Between the ages of 4 and 11, I suffered from epilepsy and had a lot of trouble with all of the medications the doctor put me on.
Could this predispose me to an eating disorder?

I was "actively aneroxic" for about 2 years when I was in junior high/high school and once reached 85 lbs at 5'4". I did not seek treatment, but eventually stopped the behavior. Although my thought patterns remained the same, I have not actively restricted my food to the same degree, and am a healthy 120 lbs.
Now, however, perhaps due to some circumstances in my lfe, I find my thought patterns about my weight growing increasingly obsessive and harmful. Should I seek help at this point, even though I am not restricting my diet in harmful ways? |

My husband was diagnosed with anorexia in high school; he is now 33. He now projects his eating disorder onto me and tries to control my eating habits.
He wants to stop, but we cannot find any resources out there to help us with this sort of "projected" anorexia. Can you please offer suggestions? |

My mother is 65 and suffers from a lung disease. Over the past 6 months to a year, she has lost at least 30 - 40 lbs., and now weighs 108 lbs. at 5 feet 8 inches. She is very frail, and her lung disease is getting worse.
My mother constantly says she is not hungry, and blames her weight loss on her lung disease.
I have suspected for a while that this is an issue of control, since she typically typically eats 600-700 calories a day at most.
She is now hospitalized, and eats everything put in front of her. When asked why she is eating in the hospital, she will not answer. I think she is afraid the nurses and doctors will find she was starving herself if she does not eat in front of them.
I am terrified my mother will go back to not eating when she comes home.
Does my mother have anorexia? What should I do? |

Anxiety |

I believe I have gained weight due to a slowed metabolism and an increase in appetite, particularly a desire for carbs, caused by citalopram. Do you believe adding Wellbutrin would help with these side effects?
More generally, how can my psychiatrist treat my anxiety without causing weight gain?

My son is 23 and has a history of substance abuse, mostly opiates (oxycontin). He has been on buprenorphine for a year now and going to therapy.
He suffers from anxiety, which has gotten worse over the past few years. Sometimes he has panic attacks for no good reason, and he feels all the physical symptoms of chest pain, dizziness, and so on.
We discussed getting a good evaluation because the therapy does not seem like enough, yet he feels that with his history of substance abuse, no physician will want to prescribe medication. I hate to see him suffer with anxiety, and fear he may self-medicate in the future if not properly diagnosed and treated.
Is this where dual diagnosis comes in and who would be the best physician to see? |

Binge eating disorder |
| |
 |
I am a 56-year old divorced mother, and my daughter went away to college last fall. A few weeks later, I lost my job, and have been only sporadically employed for almost a year.
I have had ongoing depression related to both of these losses, as well as insomnia and panic attacks. I have had a weight problem for most of my adult life, and recently have found myself eating continuously without feeling I have any control - to the point where I am full to the point of feeling sick. I recognize the symptoms, and am concerned that I am relapsing into binge eating, which I suffered from as a young woman.
Is this sort of relapse after many years common?
Are their treatment centers that will take my COBRA insurance? |

Bipolar Disorder |
| |
 |
I am a 53 year old woman with bipolar I disorder. Although I had my first episode at 15, I was not diagnosed until 7 years ago.
Over the last 7 years, I have been treated with all the available medications and protocols, but have found no lasting relief. My current doctor says he has nothing else to offer me and suggests I just wait until new medications become available.
I do not know if this is possible. I have attempted suicide in the past, and with no medications on board now, I am feeling really bad and hopeless.
Do you have any other suggestions besides just waiting?

I have an ex-boyfriend whom I think has had schizophenia or bipolar disorder for the past 3 years. He's not himself. He hears things that only he can hear and sees things that only he see. He lost all his friends because he thinks his friends are all in on getting him.
He sees a doctor, but he's not on any medications. I think that's because he's not telling the doctor the whole story. I feel very bad for him and I feel sorry for his mother. Is there anything I can do to help? |

I was evaluated by three different professionals. Two of them said I had bipolar II disorder and one said I had no signs of that illness. Due to the disagreement, a fourth professional diagnosed me based on the three previous assessments. This person never met me, so I am wondering whether his opinion can be accurate. Also, is it common practice for diagnoses to be made without examining the patient? |

Is there any recommended method to minimize risk in multiple-time zone travel for bipolar patients?

My daughter, 17, had benign occipital epileptic fits at age 4, which stopped with Tegretol. Since her school days, she has been aggressive, even abusive. She also has episodes of weeping that suddenly change into silly laughter and euphoric mood. Mood stabilizers have no effect and when she was taken off risperidone, she became unmanageable.

Two weeks ago, my wife and I caught my daughter, 8, masturbating. We first have since found out that she has been doing this every day for the past month.
We interrogated her to try to find out where she learned how to do that, but she will not give us any answers. Should we not have interrogated her? What should we do? |

I am a nontraditional student returning to college with diagnoses of depression, bipolar type II; ADHD; social phobia; and another anxiety disorder due to a trauma.

How should I approach finding someone familiar with helping people adjust to academic environments? |

All of the articles I have read about the supplement SAM-e recommend the butanedisulfonate form.
However, all of the SAM-e supplements I have seen sold at retail do not use that form of SAM-e, but rather the 'tosylate form.'
Is the 'tosylate form' really better? Why?
Also, where can 'butanedisulfonate' SAM-e products be found? |

Can a drug change who you are or how you feel about somebody? Specifically, does the use of SSRIs result in apathy and a lack of positive emotions, such as love?
I have taken Prozac for 15 years and I am now switching to Luvox for OCD and depression. I have felt "blah" for a long time and still do. Something I do not even know what I am feeling.
Is there a way to counteract my current state? I have heard about the possibility of adding Wellbutrin, but I am worried that doing so may increase my anxiety.
What do you advise? |

I am 25 years old and have suffered some form of depression and anxiety since I was ten. I have been in psychiatric care and taken many medications, but nothing seems to work. I have a constant headache on the left side of my head, and I am exhausted most of the day because of sleep disturbances. I have sleep paralysis at least once a month and feelings of deja vu before going to bed.
What kind of professional should I see? |

I am 63 and have had psychotherapy and taken the antidepressant imipramine for a long time. My psychiatrist thinks I have psychosomatic depression. What is that? And if my doctor adds additional medications to my regimen, will this cause restless leg syndrome? |

I had been suffering from depression for almost 10 years until 3 years ago, when I finally sought help.
I started taking Lexapro, which helped me enormously until the last 6 months, when I started again having the same black thoughts and heaviness that I had during the years of depression. Is it possible that my body got used to Lexapro and I should change the medication?
Also, would you consider this depression an episode that can go away, the same way that it came? I am very scared that the years of depression are coming back and I feel incapable to go again through this suffering.
Finally, should I seek additional help? |

I have started taking Rhodiola and am feeling the benefits of it.
What is Rhodiola and why does it work?
More importantly, I have read that if you take this drug continually, you can lose its benefits, so it is best to stay on it for 6 weeks on, go off for 2 weeks, and repeat the cycle. Is this true? |

I know that my loneliness is a major cause of my depression, but the depression makes it much harder to reach out. Similarly, I know my unworkable job contributes to my depression, but I am in survival mode and can't organize myself well enough to change my situation. It seems like a vicious circle, and none of the innumerable medications I have tried over my 60 years has helped so far. How do I break out of the cycle? |
| |
 |
I recently started taking 30 mg of cymbalta. I have always taken extremely low doses of antidepressants because I am medication sensitive.
I am overwhelmingly tired on 30 mgs. and the doctor says I should go up to 60 mg. because the tiredness is depression. I do not feel depressed, I feel sedated.
I think I need a smaller dose, not a higher one. What do you think? |

I retired from the New York City Fire Department and find myself obsessively buying and collecting guitars (I play), knives, rifles for target shooting, and magazines. I always buy too many of what ever it is I'm interested in at the time. I also dive head first into learning things to the point of being ridiculous.
Do I have obsessive-compulsive disorder or something else?

Dysthymia |

I am 46 and have been suffering from dysthmia for 16 years. I've tried lots of antidepressant--SSRIs, SNRIs, and tricyclics--and what happens is that after I try a new one, I feel better for about 3 months and then go back to feeling tired and depressed.
Is there any new medication or combination of medications that might help me? |

Family issues |
| |
 |
My current girlfriend has some serious insecurity and jealousy issues. This causes anger and leads to physical and emotional abuse and my children are suffering from the fighting.
My girlfriend finally realized she had a problem and is going to examine whether it is hormonal, emotional, or chemical. I want to leave, but she wants me to solve this problem with her.
How effective are hormonal or drug therapy in such situations and how long does it take? |

My oldest sister, 63, has claimed to know more than doctors for years and has dabbled in natural health remedies.
In the last few years, however, she has begun to seriously "diagnose" illnesses in others, usually less informed people who are needy, and some people are not taking prescribed medications, or are taking over the counter medication at a much higher dosage than recommended, at her instruction.
Why does she do this? |

I have been told my friend, 14, has a personality disorder. She has trouble keeping relationships because she constantly makes up stories and rumors about people, often of a sexual nature, and she sometimes seems to believe her imagination.

Is there anything I can do to help her without getting harmed? How would I go about getting professional help for her? |

HIV |
| |
 |
Am I being a hypochondriac? A homeless man shook my hand a couple days ago because I gave him some money. Afterwards, I got scared because the day before, I got a superficial cut about a quarter of an inch long on my index finger. I didn’t see any wounds on the man or fluids on my hand, but I think the man was a drug user, perhaps because of his teeth and the burn scars on his hand. I got home 15 minutes later and washed my hands with peroxide and alcohol, worried that I might be at risk for HIV. If this man was HIV+, what are the chances that I am now HIV+? |

I have recently received oral sex from my girlfriend, who has been sleeping around about 2 to 3 times a week. All we did was oral sex without a condom.
Am I at risk of getting HIV? Should I be tested? |
| |
 |
Last month, I had sex with a woman. After about 10 minutes of intercourse, she took the condom off, performed oral sex for about a minute, and put the used condom on inside out. I think her vaginal fluids must have been on the condom and now I am scared that if her vaginal fluids contained HIV, I could have been infected.
How likely is that?

Last week, when I was drunk, I had my first, and very unexpected, sexual experience with a male friend. We did not use protection. Should I get tested for HIV? And where do I go from here? I am in love with my wife and cannot imagine telling her what happened or suggesting we use condoms when she is on the pill.

My brother recently told me that he is HIV positive. He found out in January and his doctors told him that he won't be put on medication now and that he should try to maintain a healthy immune system by exercising and eating dark greens. My brother, however, also suffers from depression, but he is medicated. Unfortunately I don't think his medication is working and on some days he can sleep as much as 18 hours in a day. Could his depression affect his immune system and speed up the progression of the virus? |

Medication side effects and toxicity |

My mother has been taking Ritalin for her adult ADD for more than 10 years. This afternoon it was found out that she just had a minor stroke and has evidence of possible prior strokes. Strokes do not run in the family, she is not elderly and has no history of heart disease.
My question is twofold. One, could it be possible that her diagnoses of ADD was really due to tiny strokes and not an illness in and of itself?
And two, could stimulant medication be making whatever neurovascular condition she has worse?
Her internist has stopped her Ritalin and doesn't want her to ever take it again. When will psychiatry do more than just treat the symptoms? |

I began dating a veteran of Afghanistan about three weeks after his return. About two months into our relationship, my otherwise affectionate, attentive partner has begun to exhibit symptoms and believe that I am the cause of all our problems.
When I tell him I need reassurance about our relationship, he gets angry and avoids the topic entirely.
I am willing to stand by him through this, but I do not know how to communicate my own concerns. |

I am a therapist for families that are so dysfunctional that Child and Family Services has intervened on behalf of the children’s welfare.
Recently I saw a woman, 31, with an eleventh child is due in September. Several of her children have been removed from her care; those in her custody are habitual juvenile offenders. She has been charged with child abuse more than once, and is now incarcerated after being charged with torture and willful abuse of a child under the age of 18.
Is some emotional need being met through these pregnancies or by “being pregnant”?

I am 18 year old boy from the Georgian Republic. Since the death of my closest friend six months ago, I have been suffering from terrible and obsessive thoughts and phobias about death, suicide, cardiac attack, and so on. I also have dizziness, heart palpitations, insomnia, chest pain, sweating, chronic fatigue, and body tension.
A neurologist told me to take Valium and Lexotan, and thanks to this treatment, I am feeling better, but I atill have psychological problems: I do not know what to do, I have lost my self-confidence, and I have compulsive ideas.
I never drink, use drugs, or smoke, and none of my family members have mental disorders.

My adult son has had schizophrenia since he was a teenager. Currently, he is taking 175 mgs of clozapine a day and is happy on it. Before he took this dose of clozapine, he was on a larger dose of Clozaril and had side effects.

My daughter's father has schizophrenia. My daughter has a speech delay, poor eye contact, and so on. Should she have an evaluation? And is there anything we can do to ward off the illness? |

My son is in the prodromal phase of schizophrenia. Several psychiatrists have suggested that Clozaril is the best drug to treat schizophrenia even though there is a 1% to 2% chance of agranulocytosis, and thus drawing blood to check for this potential problem is neccessary.
I've done quite a bit of research and it does seem that Clozaril can be more effective. Why is this drug not used more in the U.S.? |

My son, 32, has disorganized schizophrenia, but his complex denial leads him to believe he has a spiritual maturity problem. He will often, but not always, answer even basic questions, such as, "What is your address? Name? Phone number? Date of birth?" with "I do not know" or "I do not remember."
Is this paranoia, an attention problem, difficulty in the speed of processing thoughts, some form of learned helplessness, or something else? And what is best strategy for dealing with it? |

How do mood and anxiety disorders increase the risks of dementia? |
| |
 |
My daughter has been diagnosed with anxiety. She is 16. She is presently not attending school. She is able to go out with friends and sometimes go places with our family. She stays in bed until 1 or 2 in the afternoon. I feel her biological clock is messed up.
She had trouble leaving the house when she was 9, she did go to school only. Then again in 7th grade she stopped going to school at the end of the year and returned to school in January of 10th grade. She was great. She had a 3.8 average all of freshman year and she was weaned off prozac . Last Feb when she got onto 10 mg Prozac she couldn't go places, because she was anxious again. She tried to go to school but could not. As they increased the Prozac again, nothing worked and at 40 mg, she became manic.
Now she is on Effexor, and seems good but cannot get back to school. She has home instruction but there are days she won't get out of bed and doesn't know why. Some days she can't go, and I think it is because she didn't understand an assignment. Other days I think OCD thoughts keep her in bed.
How do we help with her biological clock? Should we use melatonin? Should she be reevaluated? She wants to go to school but doesn't know why she doesn't go.
Right now we need a therapist too her txpist is moving. Do you think she needs to go to a hospiatl for intensive therapy? She isn't suicidal , she does not use drugs or alcohol. This is a puzle and we need guidance. Thank you. |

Borderline personality disorder |
| |
 |
My daughter is 23 years old and has been diagnosed with borderline personality disorder. Another psychiatrist has diagnosed her with bipolar.
Regardless, she has had two suicide attempts, been on every medication and combination known to her doctor, and is now going through ECT for the second time. Nothing seems to help.
Are there any other treatments or facilities that can help her with the depression, anxiety, weight gain, severe head aches, etc? |

Child and Adolescent |
| |
 |
My 16-year old daughter can be fine and then suddenly get upset for anywhere between 5 minutes to an hour before returning to normal. She also gained 18 pounds in 2 months, perhaps because a holistic doctor prescribed 2,400 – 3,6000 mgs of fish oil daily for mood swings. Are her moods and weigh gain related to hormones or her refusal to eat enough protein or something else? Is there a blood test or hormone test that you would recommend? |

After 2 months of treatment of depression, I m feeling improvement. I'm taking Paxil 40 mg (paroxetine) in morning and Risperdal 1 mg (risperidone) along with Klonopin 2 mg (clonazepam)at night.
This combo effectively reduced the symptoms of depression, but I'm not feeling enthusiastic and I want to elevate my mood.
I want to know whether bupropion or methylphenidate can help me feel enthusiastic or elevate my mood? |

My son attended a rehab center for substance abuse at the age of 14. He is now 16 and is involved with AA and is seeing a therapist. He has been sober for over 2 years. My son will be 14yrs by April 2008. Since the last few months, he has had fears of death. He hates the word "TERRORIST" and is afraid of terrorism. He has the phobia that soon we will all die, and he will be left alone. Previously, he was a lively child surrounded with friends and was a fun loving boy. Now since the last 5-6 months, he prefers to be alone and likes to be in his room and does not want to mingle with any of his peer groups. On the other hand, he prefers to be with boys elder to him. He is not interested in studies anymore and is not participating in any sports or extra curricular activities.
I am genuinely worried about my son because now I find him totally a different individual. Please help me out. |

Dysthymia |
| |
 |
"Do I have dysthymia? I read an article on dysthymia and started to think I might have this condition. I think most of my friends would describe me as a really nice, guy, easy-going, pretty social.
In general, I operate at a low energy level. Sometimes I feel like I totally lack emotion. My memory in general isn't great. It has probably been five years since I've woken up and really felt energized and ready to go.
I remember I used to have days like this--maybe six times a year--and really thinking it was strange that I felt so good. This was my first clue that I was maybe not feeling the way I should. When I was younger, in grade or middle school, I remember crying over simple things, and feeling lots of anxiety over performing in school.
Drugs are a big turn-off for me and the thought of talking about my feelings to a therapist isn't appealing at all to me. Any advice would be appreciated."

Eating Disorders |
| |
 |
A female friend, who is 5' 2 "tall and weighed 103 pounds, has lost 20 pounds over 2 years. Her main complaint is burning in her GI tract. She believes it is reflux and gastritis, and her GI doctors call it visceral hypersensitivity.
This burning and a lack of response to traditional GI therapies have thrown her into a deep depression, acute anxiety and insomnia. She is afraid to eat because of the subsequent burning.
Nothing has worked for her. Is this an eating disorder? How do you get someone to eat if they are so afraid of pain from eating? |

I was severely abused sexually, emotionally, and physically by my first husband. Later, as a graduate student in counseling field, I had sex with my therapist when the physical attraction between us became overwhelming. I divorced my third husband while in therapy but now the therapist has told me I need to "move on" with my life and he is not interested in a relationship with me. My old feelings of being used and violated are resurfacing and I am having trouble sleeping, eating, or functioning. I also fantasize that my therapist is really in love with me and has moved people into my neighborhood and my job to watch over me and help me. Having a mental illness makes one more likely to have a medical condition. This is due to numerous reasons, genetics, stress, medications, and probably because medical conditions are frequently overlooked when your mentally ill. This needs to change!
Are there advocacy groups that fight for thorough medical workups for all of the mentally ill? Psychiatrists need to instead of just focusing on trying to find the perfect cocktail of medications for their patients, or delving into strange psychobabble ideology; incorporate other fields of medicine into their treatment regiment; especially, when previous treatments have proven to be unsuccessful.
Psychiatrist should be well rounded physicians especially in fields that can effect mental health like neurology, infectious disease, gastroenterology, cardiology, etc. The treatment of the mentally ill needs to change. |

Smoking Cessation |
| |
 |
I successfully quit smoking (decades-long habit) a full year ago; I have recently begun experiencing dreadful tastes with every breath I draw. I can best describe these tastes as "wet ashtray". Mouthwashes of various types, extreme brushing of teeth/tongue/reachable areas of throat do not help, nor does eating strong-flavored foods.
My medical doctor has never heard of this and offers no suggestions. Any insight you can offer would be much appreciated. |
| |
 |
I was told by a psychiatrist once that smoking would help the kind of depression that I have. So, I tried to get rid of my depression by chain smoking. Instead of helping, my depression worsened and I developed a pulmonary condition. I guess he doesn't subscribe to the doctrine "above all else, cause no harm" . Do you think that psychiatrists should tell their patients not to smoke? Or do you think that there are certain psychiatric conditions that benifit from smoking and outway the major and likely risk of dying from lung cancer, stroke or heart disease? |
| |
 |
Is it common for people who give up smoking to suffer increased depression or anxiety, over and above the physical cravings? I gave up five months ago, and after about a month became very depressed, and started suffering panic attacks.
This lasted about a month and gradually got better. But I've heard of this happening to other people, and it was suggested to me that it may be part of the rebalancing process of the various hormones that affect brain function. I had no other major stresses in my life when this happened, and was well over the physical craving stage of giving up, which really only lasts a few days. |
